Hi. I am trying to use your great little module to edit the entry form for an SMS form. I receive the error noted above. I've attached a screenshot. I was trying to add a bit of help text to the Phone Number field. Any ideas?

Hmm.... This certainly shouldn't happen, and I can't reproduce. Does it happen with all form fields or just this one?
