I'm looking for someone who can re-create a poll functionality (example: the poll in the middle of the sportspickle.com home page) for my site.

It must:

a. Be AJAX
b. Be customizable for CSS styling from top to bottom
c. Archive each poll as new ones are updated (example: sportspickle.com/polls)

Also, I need to customize the form that users are directed to when they want to edit their profile.
