"Video (open package)" still in Create Content menu

I installed bd_video and all all other required modules on which it depends, I followed instructions on this page:
http://drupal.org/node/421120, but when I go to "Create content" I see only "Video (open package)" option instead of Blue Droplet or bd. Is that how it supposed to be. First I thought that this is because I had two MySQL tables left after op_video was uninstalled in D5, but now I upgraded to D6 and these table were still there. I then uninstalled BD video and removed those two tables manually. Now I re-installed BD Video and still get "Video (open package)" in Create Content menu. Now when I click on that it opens form which doesn't even have any video upload field or any other video related fields.
Can someone suggest where could be the problem or am I missing something?

One other problem is that on BD Config there is a message saying that ffmpeg isn't found, although I just compiled and installed the latest version of ffmpeg on my webserver and added absolute path to it.
