Clean URLs On & Working but can't create new content or edit existing content

We have read through the Clean URL manual and it appears we have everything setup properly. Looking trough all the support issues there are many that can't get Clean URLs to work but haven't found any that deal with working Clean URLs and not being able to edit/add content.

Also.. when clean URLs are on and we click on to edit or create content we get a 404 - Page Not Found error
