I have a taxonomy term named "Academic/Honorary". Renaming it to "Academic / honorary" results in the following warning:
user warning: Duplicate entry 'category/club-type/cultural-/-international-' for key 2 query: INSERT INTO drupal_url_alias (src, dst, language) VALUES ('taxonomy/term/2', 'category/club-type/cultural-/-international', '') in C:\xampp\xampp\htdocs_clubs\modules\path\path.module on line 112.
I'm running pathauto 6.x-2.x-dev (2009-Mar-17) with setting "Update action" set to "Create a new alias. Leave the existing alias functioning", but I'm not convinced it is related to this.
My best guess is that this is occurring because while I am renaming it, the path generated from the old name is identical to the path generated from the new name.

I think I had the same error when trying to change the order in my terms list. I got per term the following warning:
user warning: Duplicate entry 'taxonomy/term/2-' for key 'dst_language' query: INSERT INTO d6_url_alias (src, dst, language) VALUES
('taxonomy/term/', 'taxonomy/term/2', '') in /opt/www/vs01009/web/mullion.chilli.be/modules/path/path.module on line 112.
Then I took a look in my database and found that there were indeed duplicates of each of my terms. For each term I had a record that the path module would make and after each record I had another record the pathauto module would make.
So I guess there's a problem at creation of the term, when pathauto should be taking over the INSERT job but is failing to do so. Only when trying to update the term the code finds it's way to the pathauto module.
That's how I understand the problem. But I have no clue how to correct this...

aimevpAfter better observation I saw that it makes both aliasses at the same time...
pid src dst
1 taxonomy/term/ taxonomy/term/1
2 taxonomy/term/1 pathauto/alias/to/my/first/term
3 taxonomy/term/ taxonomy/term/2
4 taxonomy/term/2 pathauto/alias/to/my/second/term
etc...
I hope there's someone who can help us.
Btw, maybe it is related to pathauto after all...

woodbutcher commentedI'm getting the same error, using pathauto and taxonomy manager - I think the error showed before installing taxonomy manager as well. What problems might the duplicates cause, should I remove them as I go, or when I've finished adding terms? The site is not live as yet, just in development.
