Recover direct push updates after missed events or reconnects

Problem/Motivation

Clients applying pushed updates need to detect when changes were missed. An opaque generation token does not establish event continuity.

Proposed resolution

Define continuity and duplicate-handling rules. Replay missed updates where available; otherwise reconcile through HTTP.

Remaining tasks

Implement and test interrupted streams, missing events, duplicates, expired history and account changes.
