Replace load-all-completed-orders iteration with SKU/order-item query or bounded cache for large B2B accounts.

Proposed resolution

Entitlement check scales with product/SKU index, not full order history load. Kernel test for multi-order case.

Fixed on 1.x (merged PR #48). Confirmed complete in issue/doc sweep 2026-07-31.
