[1] site/default - directory CHMOD
[2] site/default/default.settings.php - file functioning
[3] site/default/settings.php - file creation
[4] site/default/files - directory creation and CHMOD

This module has been deprecated according to the project page. see: http://drupal.org/project/installator
Thus it should not be used as it is no longer being developed.

if you have mistakenly placed this issue in the wrong project please place one in the correct project. To me it looks like you are trying to file an issue against drupal core.
