Rename the form classes

The form class names should start with UserRestriction to match the configuration entity name implemented by the module. That is how similar classes are named in Drupal core.

Those are internal classes, so they should not be used by other modules, and there should no need for the change to be backward compatible (although, if possible, that would be a positive point).
