There is a duplication of action buttons on delete taxonomy term modal form.

Steps to Reproduce:

1. Install the module.
2. Navigate to the taxonomy vocabulary overview.
3. Click on "Delete Taxonomy Term."

Expected Result: The modal form should display a single set of "Cancel" and "Delete" buttons.

Actual Result: The "Cancel" and "Delete" buttons are duplicated in the modal form. See attachment.
