When I create a menu item directly from the menu edit page, any permissions I apply to it are not recognized. However, if I first add a page and create a menu item from that page, the menu item can be properly assigned to roles. I have nodeacess installed as well, so I'm not sure if this has an effect. I'm looking into this, but if anyone else has input, please share!

This issue can be fixed using the directions here: http://drupal.org/node/234278. I used nice_menus as well.
