Update Session Submission Form

We need to add fields to training and session submission forms.

New Fields:

• Is speaker's attendance is dependent on the acceptance of their session?
• If a session has been given previously? Was it recorded? How many times has it been presented? Answers to these questions would not disqualify a session, but would be helpful in building a balanced schedule.
