At the moment the weight of the $form['read_only'] is 0. This makes the fieldset of the module on the maintenance appear between the checkbox "Put site into maintenance mode" and the text field of the message for the maintenance mode.

My suggestion is to change this value so that the fieldset appears at the beginning or at the end of the form.
