No Title Field as in Screenshot

I realize this module is not maintained, but I'd like to ask a question at the very least. The screenshot shows a "Title" field above the "Link to" field. I've installed the module and the Title field is not there . Is that functionality provided by this module or by another one? Is there an easy way to add this functionality? This is a crucial element for my client. I'm relatively new to Drupal. Thank you.
