Report missing JS on certificate generation form submission

The certificate generation form should not be submitted to the server as the handling should be done completely by JavaScript. However, we should tell the user if JS isn't enabled via a form submission handler/page. That is, if the JS doesn't take over the form handling, the user will see this message.
