It appears that the NAME variable isn't matching up. You create the DM or QR code and the name is the default "DM" or "QR".
Bug confirmed.
New dev version will be available later today.

Download the latest Dev build and it should work properly.
I un-activated mobile codes, downloaded the 6.x-1.x version, unpacked and activated it. I ran Cron Job to refresh.
Here was my test post: [mobilecode type="qr" data="url" size="small" name="test_005" tinyurl="0"]Visit xx Soon[/mobilecode]
here is the result of a test post:

Did I miss something? or, not refresh the module correctly?

Did you download the 6.x-1.x-dev version?
The output your getting looks like you are running an older version of the module, maybe 6.x-1.0?
