Drupal.org has continued to have instability related to opcache corruption, even after the PHP 5.4 upgrades. As part of our ongoing investigation, we are improving the health checking of the individual web nodes.
The health check should validate that access to the filesystem, database, memcache and solr are functioning. Additional checks into the health of the opcache would also be beneficial, and will be implemented if we can do so.
Production outage: No - individual web nodes will be taken out of rotation as needed to implement changes.
