Clean up functional_content usage / detection

1) Use functional_content_nid() instead of the hack-ish _dvg_appointments_is_appointment()
2) Detection if a Webform is an appointment creation form should be done through $node->webform['appointment_enabled'] instead of functional_content_nid()
