When in Activities tab you choose Activity Type "Petition Signature" the field "Survey/Petition" appears. But that fields only contains "-User Select-" and "-None-".

The active petitions do not appear.

I did some investigation and found the cause of the problem.
Filed as https://issues.civicrm.org/jira/browse/CRM-16236 and fixed with https://github.com/civicrm/civicrm-core/pull/5567.
