I have updated CKeditor on a site from 4.0.1 to 4.3.1 and all buttons disappeared after the upgrade. Several cache clears have not helped anything. It looks like the Expire header set's one month for all JS files and causes this. As the foldername and filenames have not changed for the files the changes are not reflected on client side.

The bug is here that Drupal is using cache url parameter and change this one cache clear and I guess CKeditor is not updating this value to circumvent browser client caches. I have no other idea why this happens.

After manually clearing the browser caches all buttons re-appeared, but I cannot explain this to thousands of users and they also do not like to wait 4 weeks until their editor recovers from this bug.

Sounds like a dup of #1956316: Toolbar buttons loaded with wrong icons and #2147155: Icons are broken with 4.3 library
