Incorrect insert/update logic in save() for cache and hooks

Salsa might decide to update even if we think it is a new record.

This patch updates the cache logic to fix bugs related to re-saving of old values but we should actually somehow allow the API to return the update/new flag (by reference argument?) to also fix the hooks.
