I'm inclined to won't-fix this— the custom permissions show up on the Permissions page after they are set, so it's not all that intuitive that the custom permissions should be a tab next to Roles on the permissions page.

Probably a page in the admin/config space makes the most sense for this module, but i'm not feeling compelled to change it one way or another.

What about at:
/admin/config/people

For Drupal 7

It's configuration, so it kind of makes sense.

admin/config/custom_permissions ? Or you mean config under people? Now i'm inclined to go with your original suggestion... 'Custom Permissions', 'Permissions', 'Roles'... why not...

It makes sense because its configuration, not the actual permissions.

This section only has normally 2 items: Account settings & IP blocking.
