This is a good start on creating a single widget that can provide combonation widgets like a yes/no other: (text field) or a series of questions with Yes/No/Don't know inside of a single table. Doing this manually with css is a major hassle.

Is anyone available to create such matrix widgets? If so let me know and we can talk $.
