This was needed in our case as we have a page node set to front.
Sorry to post it like this but I didn't know what else to do. If you could let me know the preferred way I would be more than happy to follow it in future.

We’re working on getting Token integration into 5.x-2.0. That work should already allow putting [page-title] into the home page pattern, so we won’t need your patch.
But I will make sure that your functionality gets into 5.x-2.0.
