Error on date field [#1666340]

On date field is not well defined #name, #title so admin prodice two PHP 5.3 notices in Drupal Error HTML place.

Notice: Undefined index: #name v clientside_validation_regular() (riadok 197 z ...modules/clientside_validation/clientside_validation_form/clientside_validation_form.module).

I added test for text of existing variables, because, what can show Client side validation if this filed have no name and title?

Possible related to #1669570: Date field validation

Can you post a screenshot of your date field on the node/edit page?
What is the default value?
Is the field required?

I have the same issue here.

Notice: Undefined index: #name in clientside_validation_regular() (line 194 of xxxxx/Sites/acquia-drupal/sites/xxxxx/modules/clientside_validation/clientside_validation_form/clientside_validation_form.module).
Notice: Undefined index: #title in clientside_validation_regular() (line 194 of xxxxx/Sites/acquia-drupal/sites/xxxxx/modules/clientside_validation/clientside_validation_form/clientside_validation_form.module).

The field is required and the issue stops if not required.

I attached a screenshot of the date field edit page.

Any help would be appreciated :)
