Per country postcode filter lists

Currently the module is only geared up to cater for a single country at once. There is a global filter mode and filter list. However, for a store set up to deliver to multiple countries, this would not be sufficient (E.g. there would be collisions between UK postcode prefixes and US zip codes). Instead, there should be a filter mode and list for every country enabled on the store.
