I installed v5.x-1.1 and then updated to the dev to see if any different. Images simply open up directly in a blank page.

I'm using CCK, Contemplates, CCK Imagefield, Image module, Imagecache.

My Lightbox options are:
Use Lightbox2 Plus - On
Enable for Image Nodes - Off
Enable Grouping - On
Disable Lightbox for Gallery Lists - Off
Enable Gallery 2 Filter - Off

Tried different options - still can't get it to work correctly.

I think you've downloaded the scriptaculous and prototype files from the incorrect place. You need to download them from http://www.stellapowerdesign.net/scriptaculous-drupal.zip.
