Merge fields to display settings in user settings page into a single table

The user setting page would look better if it was compacted a little. Right now too much space is taken up be the display field checkboxes. As a result the other settings options on this page become a little lost. I suggest merging the checkboxes into a table with the field names as row headers and tab/page name as column headers.
