Operations column in report tables, shortcut to add path redirect [#1205456]

I've written a patch that adds a new "Operations" column to all report tables.

I also added a "Add redirect" shortcut as an operation. I added a form_alter for path_redirect_edit_form that prepopulates the source field with the path of the 404 record.

This has been committed, with a couple minor adjustments:

Removed the change to the pager query that was meant for #1205242: Page counter query returning wrong value for referrer page
In the Operations column, I added pipes to separate the Edit/Delete Redirect links to make it clear that they are separate.
