Are there any plans to support Survey under Drupal 5.x? How soon might this happen?

Have you looked into CCK and Views as a similar replacement? You could create a 'survey' by creating a new Content type and then group together all of those nodes using a view. It's not ideal and not an exact replacement, but if it suits your situation you'll also get all the benefits of having the 'responses' as nodes.
