Additionally, the add condition page title is "Add a new condition set (AND)". Basically, it looks like it's using the label of the parent object instead of the new object itself.

Indeed, I've committed a fix for that.

Unfortunately I had to use a weird workaround to access the pluginname - as we are already hitting the menu-path-part limit of 10. Anyway, it works.

hah, I was wondering what happened to that limit... I remember 7 parts being too low on D6, but I don't know why they changed it to the equally-as-arbitrary limit of 10. : P
