Image cache thumbnails are not both deleted on file replace [#671090]

Only 1 of the cached imagecache images is deleted when a file gets replaced. The cached image will stay arround until you flush the imagecache cache.

I suggest if you upload images and use file revisions that you flush all your imagecaches.
