Thanks for the module, it is just what I needed!

Permission text on top of the form is triplicated; one will do. Further the text mentions 'above bank account' but the account is below. Just leave out 'above' and its fine. See attached screenshot.

I'm not sure about the check box being required or not. AFAIK this check box can be removed from the form on the admin page right?

The text should indeed change to a more position agnostic form.

I changed the text to 'given bank account' instead of 'above bank account'.

I agree it's is a lot of 'agreement' but is see no 'generic/easy' solution. ?!?You could change the translations into empty versions?!?

No, translations can not be empty. I use hook_form_alter() to reduce the amount of text, which works but is far from an optimal solution :(
