List of Security Qsts in backend are missing except most recent

Please let me know if you need further info, I've never posted a bug before..

Password reset had a default of "library card number", I added "Mothers Maiden Name", to which it then listed 2 items in the "List" (to edit or delete)

Then I deleted the "library card" one, and added 3 more.. "Father's City of Birth", "Mother's City of Birth", and "Favourite Pet".

Each time I add one, the list says that it's been added:

"Security question Name of favourite pet? has been added."

but doesn't show up in the list.

And when I try to register as a new user, all of the options show up, so they're around somewhere in the database..

But back on the list page, all are missing, except the most recent.. ie:
Name of favourite pet? 0 Edit Delete

I cannot delete or edit any of the existing options that I previously added.. and unfortunately, because of this, I added 2 of the same because they didn't come up in the list, so now have 2 x Father's City of Birth and can't delete the duplicate.
