Feature:
- added caching support
- add optional quail library patch reduce-pNotUsedAsHeader-false_positives that helps reduce a significant number of false positives with short bold paragraph sentences.

Bugfix:
- #1401828: Database errors upon installation
- #1401844: Line/element information not displayed
- The 'total' for all tests was never being incremented from 0.

Cleanup:
- cleanup syntax and try to follow drupals coding standards more closely.
- fix some spelling mistakes.

Important Notes:
- The line number reporting needs significant reworking, see #1437964: Remove or hide line number references.
- That issue may/maynot require changes in this module in addition to the changes in the node_accessibility module.
