Label radios/textfield element with prefix Quiz

Problem/Motivation

For me, it was hard to find out, how do I add/change the element until I saw the element had the same label.

Proposed resolution

Prefix the element label with Quiz as it was done with Quiz Select #3083870: Add support for select element and Quiz Checkboxes #3083782: Add support for checkboxes element.
