Assestment element

I'm looking the way of making a new option element on webform 8. I'm coming from limesurvey and its assestments for making scores.

Any suggestions on making an option element like. I'm trying with composite element by I don't know how to manage.

A1 0 Never
A2 0 almost never
A3 0 smetimes
A4 1 almost always
A5 2 always
