Problem/Motivation
During a recent discussion on the Google+ Drupal Community it became apparent that it is not clear from the token module page that there is a D8 version and it provides a UI for browsing tokens.
Proposed resolution
Add the D8 token module issue info text to the token module homepage.

Under a suitably titled heading such as "Token for Drupal 8", something along the lines of the current D8 issue text:
"Provides additional tokens not supported by core (most notably fields), as well as a UI for browsing tokens."

BerdirI'm not sure that such a description would have actually helped that specific person.
But, yes, the project page really needs an overhaul. Just adding a bit of D8 text doesn't really cut it. Looks like it's still written from D5 perspective, with a "oh, there's also a cool new feature in D6" part ;)
Note that the module offers pretty much the same for D7 and D8, no need to make that D8 specific.
Want to help with this? We should at least..
* Replace the whole initial chapter with a chapter that starts like yours, we can still expand it later, possibly add a list of common tokens that are provided
* Add a Drupal 8 chapter, that explains the current status
* Remove #D7CX, not sure if we want to keep the accessibility thing.
* Provide a screenshot of the token UI instead of the current one.

"Provides additional tokens not supported by core (most notably fields), as well as a UI for browsing tokens."
...then go into what tokens are for anyone who doesn't know, perhaps adding a recognisable example - the current text feels as you say old.
dictionary.reference.com says "something serving to represent or indicate some fact"
Got to get back to code but will continue to ponder, feel free to change what you can on the module homepage for the moment,

The description is quite short, but I'm not sure if we really want to say much more. Better have it short than wrong I think. More details could be in documentation, which is probably equally outdated.
