Sticky Notes Not Visible

Running Drupal 6.19 with Garland theme.

Sticky Note: 6.x-1.0
jquery_update: 6.x-2.x-dev
jquery_ui: 1.7.3 (module 6.x-1.4)

I managed to get the modal dialog to appear and show the sticky_note form, but after submission, the dialog fades and the sticky note does not appear. The status in the bottom states '0 notes'.

Upon going to the sticky note overview, the note is there, but the path is blank. When I edit, the radio button is not selected and only shows 'show on all pages' as an option.

I've tried multiple versions of jquery_update, jquery_ui, and sticky_notes without success. I've also tried multiple permissions (enabling them all) with no luck.

Even for user 1, number of notes always shows '0'.
