ShURLy - http://drupal.org/project/shurly is a fast base to build a unique URL tracking system on top of. True tracking comes at the cost of a cookie and usually some JS. This all comes at the cost of a page load. So you want a cookie or not?

One can inject valid html head code like js into the admin form at http://shurly.localhost/admin/settings/shurly_pageload. The page is recreated every time new code is added.

As all of this is running at boot and the page that is loaded is a non-Drupal page it comes with little extra overhead and is very quick.

Although not really perceivable to the user, a page is loaded with a URL query string that passes the destination and the source. These will come in handy for tracking.

Pretty much this exercise is complete except for the polish.
