The theme function is being bypassed because it's not running. And it's not running because it's not registered. It needs to be registered according to Using the theme layer (Drupal 7.x).

Registered theme in f5fb5ef by adding a hook_theme() implementation. The formatter is now properly going through the theme function to display the field contents.
