Wrong user selected when having duplicate realnames

My realnames are firstname + lastname. As a results I can have duplicate realnames for users that register on my site.
When selecting the correct user in my user reference field (Autocompletion JSON), it picks finally another user who has the same realname to submit. And it saves another user with the same realname in my user reference field.
How do I handle duplicate realnames?
