I think the main difference is that profile_manager makes "optional modules" available to users. The user can choose to enable them or not. I think with config_profile enabling functionality on a site is more of a developer or site builder task. profile_manager does a lot more like giving users a customized admin toolbar and stuff, but allowing users to enable "optional modules" has gotta be the biggest difference.
