Perhaps I am missing something or one of my modules is causing this...

But the only time I see titles is on content generated by Devel.

It's important to note that the titles I saw were DIFFERENT then the titles I would see when on the find content page.

There must be a logical reason, thanks for any help.

You have to enable title in the image field settings:
Manage fields -> edit (of your image field) -> check the enable title checkbox
