The buttetin upload page is broken, the fields are misaligned.

The installation is new and clean running v7.x.2.1.

A screenshot of the issue is attached.

Is this a known issue?

Ran an upgrade where the core was upgraded from Drupal 7.36 to 7.38 and the install profile changed to openchurch-7.x-2.1-beta7 from openchurch-7.x-2.1, yet the problem is persisting.

Any suggestions on how to fix the issue?

This is very strange. It looks like your edit page is trying to use the OpenChurch theme instead of the admin theme (aka 'seven'). Are you logged in as the super admin? I could not reproduce this issue.

Please provide more information. Maybe there was a problem with the install? The only other thing I could see is a permission problem if you aren't using a different user role to edit that page.
