If I'm viewing a site in Spanish, and come across a language neutral page (I'm at that point on /es/patients-families/just-kids, the multilink redirects me to /patients-families/just-kids), which of course is bad because the subsequent links will be in English. Any way to fix that, so language neutral node links aren't affected?

Hmmm.. I may have misunderstood what this module does. After rereading, I see the purpose of it is to redirect to the language set by the user. I was originally thinking it was used to redirect to the language set by the URL (even if the node id were that of a page of a different language). Is my assessment accurate, and if so, is there a way to tweak the logic to look at the language set by the URL instead of the language in the user profile?

Yes, you can redirect to the language set by the url - possibly you need to change the default behaviour (in your settings.php) - see the documentation.
