Same editor but different formats?

This module is really quite awesome. However, I am running into issues in my Wysiwyg editor. For no apparent reason that I can figure out, sometimes my images are outputted as such:

Now, I am using the SAME editor, SAME content type, SAME photo, and SAME imagetype. So I am really quite confused why and how this is happening, or even how to fix it.

My first question is which format am I shooting for here? I am assuming the first, but not sure. Second, any ideas why this is happening or what I can do to provide more info?

Any help appreciated, I am completely stumped.
