A click on "Create a new captcha" brings up a page with JSON data instead a new captcha:

status 1
message ""
data
url "/image_captcha/2850?sid=2850&ts=1522504275"
token "aabfd7056324a52558108bd0858e9419"
sid "2850"

This seems to affect only Firefox on different platforms (MacOS, Linux, Android), but not Safari or IceCat, so maybe it is a bug in Firefox...

I have the same problem. So, I've checked the nginx error logs but nothing is logged.

Do you have any suggestion?

It exists on Google Chrome and Mozilla Firefox too.

I'd like to add that, the server I use has memcached installed and Drupal default/core performance caching is active.

Also, block cache alter module is active too.

I've checked both browser's console logs and nothing is logged again. Also, I've noticed that, when I click on "Generate a new captcha", it redirects me to URL below:
