Having required="true" on the hidden form input fields is preventing the form from submitting. When you select MySQL as your database and fill in all the visible required fields, you also have to fill in dummy data in the PostgreSQL fields as well to be able to continue to the next step.

pkil commentedtomz0r can we get the browser version you are using so we can try replicate it ourselves.
At the same time if you could try it with a different browser just to test.
Thanks

Also replicable on: Firefox 39.0

dr jason guo commentedI have the same problem in Chrome Version 43.0.2357.132 (64-bit) on Mac OS 10.10.3 and Firefox 39.0 on Mac OS 10.10.3. I however managed to install it successfully using Safari Version 8.0.6 (10600.6.3) on Mac OS.
