add setting for which form modes to show flag form element on, analogous to view modes for flag link

In #2759671: Flag form element shows on entity delete forms we fixed a problem with the flag form element showing on forms where it shouldn't.

As a longer-term solution that's more flexible, we should consider adding a setting that allows selecting the form modes, analogous to the setting for which view modes to show the flag entity link on.
